what surgery would give me THIS EXACT RESULT?? ughhhhhhh :/

I think i need EXXXACTLY this kind of surgery....how is it doable? what should i ask to the surgeon??? i have a visit to a plastic surgeon soon, how can i ask him to have this???? thanksss
